Patient Registrations
The way we manage patient registrations across Hollybrook Medical Centre and Parkfields Surgery will be changed between the 16-18th June. This will mean we have moved to a single patient list, rather than two separate ones.
This means:
- All current patient records and registrations at Parkfields Surgery are automatically transferred to the main Hollybrook Medical Centre list.
- Appointments, staff and services offered at all sites will remain unchanged. The difference being only the administrative structure.
Parkfields Surgery NHS app and SystmOnline users:
To continue to use the NHS app, patients registered via user ID/passphrase will need to re-register or request new credentials after the merge (18th June onwards). To clarify, this will not affect your ability to book appointments, just to use the NHS app.
Application for Online Access inc Proxy
Self-registered NHS App patients will reconnect automatically.
For new patients wishing to register with us, please register directly with Hollybrook Medical Centre.
