Clinics

Antenatal

Antenatal care is the care you get from health professionals during your pregnancy. It's sometimes called pregnancy care or maternity care.

You'll be offered appointments with a midwife, or sometimes a doctor who specialises in pregnancy and birth (an obstetrician). Majority of your Antenatal care will be at Coleman Street where the midwives are based.

Once the baby is born our team will be in touch to help you register your child to the GP surgery. If we do not hear from you within a week we will register the child on your behalf. 

Once the child is 5 weeks old our team will be in touch to get you booked in for your post-natal care and baby check.

Bloods

If you are due a blood test for any reason our staff will text you to let you know your blood form is ready to collect from your GP reception. The text you receive will also have a link to book your test via SwiftQueue. 

If we do not hear from you, we will give you a call to let you know your blood form is ready and how to book your blood test.

Cervical Smears

What is a cervical screen? 
All women aged 25 to 64 are invited by letter to cervical screening (a smear test) to check the health of their cervix. It's not a test for cancer, it's a test to help prevent cancer.

During the screening appointment, a small sample of cells will be taken from your cervix.

The sample is checked for certain types of human papillomavirus (HPV) that can cause changes to the cells of your cervix. These are called "high risk" types of HPV. If these types of cells are found, you will be invited back for further testing. If none are found, you will not be invited back till you are next due a test.

The nurse or doctor will tell you when you can expect your results letter.

When to book:
You will be sent a text when it is your time to book, with a link to book an appointment. If you do no respond one of our team will be in touch to book you in. 

In the final instance if we have not heard from you, you will be sent a letter and you can call us to book an appointment or book online.

Childhood Immunisations

Our team will contact you when it is time to book your child's immunisations. Please see the NHS vaccine schedule. 

Once your child has had the 8-week immunisations the nurse will book them in for their following appointments for their 12 week and 16-week immunisations. If you have any questions, please don't hesitate to contact us.

Contraception

We offer different forms of contraception for all our patients at the practice. We provide injections, implants, coils and the contraceptive pill.

If you wish to have an implant or coil, then you would need to contact the practice for a routine appointment with one of our GPs. They will discuss this with you further then add you to a waiting list, should these forms of contraception be suitable for you. Once on the waiting list a member of our non-clinical team will call you to book an appointment for this to be completed. 
If you require a removal or replacement, then you can contact the practice and ask them to add you to the waiting list (you do not require a GP appointment for this) and a non-clinical team member will call you to book this appointment.

If you wish to have the contraceptive injection you will need to contact the practice and book a routine appointment to discuss this with a GP. If you are suitable for the contraceptive injection, then the GP will advise you to book in with a member of our nursing team to get this administered. These injections are administered every 3 months.

For the contraceptive pill you will need to contact the practice and book a routine appointment to discuss this with a GP. If you are suitable for the contraceptive pill the GP will prescribe this for you. The contraceptive pill requires a yearly review with a nurse, or you may require a review every 6 months if your blood pressure fluctuates.

District Nurses

There are district nurses who are available to see you in your home if you are unable to get to the practice. They help with the following: dressings, removal of sutures, injections and advice on the care of the chronically sick and terminally ill. Advice on incontinence and applying pads along with other aids. To contact a District Nurse please telephone 01332 564900

Ear Syringing

Our nursing team provide an ear syringing service for our patients. You will need to contact the practice and book a 20-minute appointment for this procedure. Prior to your appointment, for the 7 days leading up to this you will need to put olive oil in your ears twice a day, once, in the morning and once in the evening. With the blockage softened, the nurse will be able to syringe your ears with minimal issue. 

Extended Access

We provide an extended access service to accommodate for patients who require evening or weekend appointments. Within these appointments we have GPs, first contact physiotherapists, health care assistants, nurses, and advanced nurse/ clinical practitioners.
We have pre-bookable and on the day appointments for our evening clinics and our Saturday appointments can be pre-booked during the week through our reception team.

Interpreter

We can arrange interpreters for any of our non-English speaking patients,  please let us know if you require this service at the time of booking your appointment.

Although the reception team aren’t medically trained, it is important this information is taken, to be given an appointment with the appropriate clinician.

It is also important that we have your most up to date contact details.

Long Term Conditions

Anybody with a long-term condition will be called in once a year for a review of their condition. You will be called in by your birth month if you are born between April-December. If you are born between January-March your birth month will be reallocated to align with our process. You will be asked to attend a community blood clinic to have your blood tests (for conditions other than asthma and COPD). If you are diabetic, you will be offered an appointment to have a foot examination. If you have asthma or COPD, you will be seen by one of our respiratory nurses.

  • Gestational Diabetes
  • Hypothyroidism
  • Pre-Diabetes

If you have more than one long-term condition, you may need to have more than one appointment. For example, if you have hypertension and asthma, you will see the healthcare assistant for your hypertension review and a nurse for your asthma review. However, if all your long-term conditions only require you to see the healthcare assistant, you will only need one appointment.

If you have diabetes, once your blood results are received, you will be booked in with one of our nurses to discuss your ongoing care plan. This can be done face-to-face or by telephone. 

If you are on medication, once your initial long-term condition review is completed, you will be booked in with one of our in-house pharmacists to review your repeat medication. You may not necessarily be contacted for this appointment, as the pharmacist will review your medication based of your bloods and questions answered. They may contact you by telephone if they have any questions. Your medication will be re-authorised until your birth month of the following year, allowing you to request your medication as normal.

Medical Reports

DWP Reports 
We process reports for DWP, these include Universal credit, personal independence payments and health assessment advisory service forms. You may require information from ourselves for you to complete your forms. All you need to request form ourselves is a patient summary. Once you have completed your form and sent this off then we receive a form to complete and send off. We process these forms within 28 days. 

Medicals 
We complete medicals for those who are wishing to foster or adopt children along with taxi, bus and lorry medicals. After we have received your forms, our administration team will call you to get an appointment booked in to complete the medical. If your medical incurs a personal charge, you will be made aware of this when you are called to get booked in.

DVLA
We process reports for the DVLA on a regular basis. If you receive a letter form the DVLA then you will need to complete this form and send this to the DVLA, we cannot complete this for you. We ca print out a patient summary, if needed, to help you complete your form. Once you have completed your form and sent this to the DVLA, the DVLA may send us a report to complete a report or a medical. If you require a medical, we will call you and get this booked in, otherwise we complete the reports for the DVLA within administration time. 

General Reports
We complete a variety of reports whether this is an insurance report, travel cancellation forms. Army forms etc. Once we receive these, we either complete these in house and send you an SMS once it is completed or we pass this to a company we use called IGPR to complete. Whether this is done within the practice or by IGPR it is completed within 28 days. If you report incurs a personal charge, you will be made aware of this. 

To whom it may concern letter 
If you require a letter from a GP then you can request this but this is classed as private work so this would incur a charge. You are welcome to outline what you require the letter to be about but the GP can add or remove information at their discretion, dependant on the information within your medical record. 

Reports we do not complete: 

  • Power of attorney 
  • ID confirmation
  • Confirmation of living at an address
  • Confirmation the patient is alive.
  • Extreme activities 
  • Court exemption

Minor Surgery

Held at Hollybrook Medical Centre

We do have a small team of GPs who can complete minor surgery within the practice. If you feel you require this service, then you can make a routine appointment with a GP to discuss this further. Following on from this consultation you will receive a call to get you booked in for the procedure. 

The minor surgery we complete is: 

  • Sebaceous cysts
  • Benign lumps 
  • Non cosmetic mole removals (interferes with day-to-day life) 
  • Non cosmetic removal of skin tags (interferes with day-to-day life)

Smoking Cessation

Local stop smoking services are free, friendly and can massively boost your chances of quitting for good. These services staffed by expert advisers provide a range of proven methods to help you quit.

One of our team will reach out to you via text to find out your smoking status. If you are considered a smoker we will try and help you quit by providing expert advice and directing you to the NHS live well which can be found on the NHS website

Social Prescribers

Our Social Prescribers adopt a patient centered approach to help you have more control over your health and focus on what matters to you. They can link you to services in the community and support you with your needs which may include:

  • Social isolation and / or loneliness
  • Emotional wellbeing
  • Healthy lifestyle choices
  • Education and learning
  • Life changing events such as birth, retirement, divorce and bereavement
  • Housing, benefits, financial support and advice
  • Local groups and activities
  • Support with long term conditions management, mental health, social needs affecting wellbeing
  • Encouraging independence
  • Befriending, counselling and other support groups
  • Assistance to complete documents or forms

We are supported by a range of community staff which include Care Coordinators, Midwives, Community Matrons, District nurses and palliative Nurses

  • If you booked via the e-referral service you can track your referral here
  • If your referral is with Royal Derby Hospital you should contact them directly by phone on 01332 340131 or via their website
  • For all other referral tracking please send us a message via our online form stating who you were waiting to be referred to

Travel Vaccinations

Our nursing team provide travel vaccination services for our patients. We require you to inform us of your travel plans 6 weeks before you depart. This means we have the time to check the vaccinations you require and to order these into the practice as they are not stock vaccines. 

Please be aware that you need to have your vaccines administered a minimum of 2 weeks prior to travel. If vaccines are administered less than 2 weeks before travel, they may not be effective as they have not had the time to get into your system fully.
